Blood sugar and your cycle: the insulin–hormone connection
If your cycles are irregular and you also live with cravings, afternoon energy crashes, or stubborn skin changes, one quiet thread may connect them: insulin. Here is how blood sugar shapes your hormones — and which labs actually reveal it.
What insulin actually does
Most of us learn insulin as “the blood-sugar hormone,” and it is — but that is only part of the job. When you eat, your pancreas releases insulin to move glucose out of the bloodstream and into your cells for energy. That much is familiar.
Insulin is also a growth and storage signal. It tells the body to build and store rather than break down, and it talks directly to the ovaries and the systems that regulate other hormones. That second role is where blood sugar and your cycle meet.
In insulin resistance, cells stop responding well to insulin’s message. To compensate, the pancreas makes more of it. Blood sugar may stay in range for a long time — but insulin itself runs high in the background, and that excess is what ripples outward.
How insulin excess reaches your cycle
When insulin runs high, it nudges several hormonal systems at once. Two effects matter most for the menstrual cycle:
- It raises androgens. Excess insulin prompts the ovaries to make more testosterone and lowers a carrier protein (SHBG) that normally keeps androgens in check — so more free androgen circulates.
- It disrupts ovulation. Higher androgens and altered signaling can interfere with the orderly hormonal sequence that releases an egg, so cycles lengthen, become unpredictable, or stop.
This is a large part of why blood sugar sits so near the center of PCOS — now PMOS. When insulin runs high, the ovaries make more testosterone while SHBG — which normally binds it up — falls. Free androgens climb, and ovulation gets pushed off schedule.
The signs that aren’t about weight
Insulin resistance is often framed as a weight issue, and it can involve weight — but plenty of women with high insulin are lean, and plenty of the signs have nothing to do with the scale. Patterns worth noticing:
- An energy crash an hour or two after eating — especially after carb-heavy meals
- Strong cravings, particularly for sugar or starch, and feeling “hangry” between meals
- Skin changes — jawline acne, or darker velvety patches at the neck or underarms
- Cycle length that keeps shifting, or long stretches between periods
- Afternoon fog, or waking in the small hours and struggling to fall back asleep
No single item here diagnoses anything. But when several travel together alongside an irregular cycle, blood sugar is worth a closer, more honest look than a one-line “normal.”
Why fasting glucose alone misses it
Here is the catch that leaves so many women told everything is “normal”: fasting glucose is often the last thing to change. The body works hard to keep morning blood sugar in range, and it can do so for years by quietly pushing out more insulin. Measure only the glucose, and you miss the effort it’s taking to hold that number down.
A fuller blood-sugar picture tends to include:
- Fasting insulin — often elevated well before glucose is, so it can flag the pattern early.
- HbA1c — a rough three-month average of blood sugar, useful for trend even when a single reading looks fine.
- Post-meal patterns — how far blood sugar rises and how quickly it settles after you actually eat, which a fasting draw never captures.

The levers that move it
The encouraging part of the insulin story is that blood sugar is responsive. Small, repeatable changes tend to matter more than dramatic ones. Think of the following as levers to investigate with a clinician, not a prescription to self-apply:
- Meal composition. Pairing carbohydrates with protein, fat, and fiber blunts the size of the blood-sugar rise.
- Movement after meals. Even a short walk helps muscles pull glucose out of the blood without extra insulin.
- Sleep and stress. Short sleep and steady stress both raise blood sugar and insulin, independent of what you eat.
- Inflammation. Chronic inflammation can worsen insulin resistance, which is why it’s part of the same conversation.
Which levers matter most depends on which pattern is actually driving your case — insulin-led, adrenal, inflammatory, or something else. That is the question we work to answer before recommending anything, because a plan built for your pattern is the one that holds.
